When Iron Tablets Do Not Fix Anemia: Reasons to See a Hematologist in Hyderabad
What to check when hemoglobin stays low after a real trial of iron.
Quick answer. Iron deficiency is a common cause of anemia, but tablets fail when iron is not absorbed, bleeding continues, the diagnosis is wrong, or another blood disorder is present. If hemoglobin does not rise after a proper trial, or the blood film looks abnormal, a hematologist should review the cause before more iron is added.
Iron tablets are the right treatment for proven iron deficiency. They are the wrong treatment when the report was never iron deficiency, or when blood loss is still happening. Months of iron without a rising hemoglobin is a reason to change the question, not to change the brand of tablet.
Why might a correct iron plan still fail?
- Doses are missed, or tablets are taken with tea, coffee, or calcium
- The stomach or bowel is not absorbing iron
- Periods, piles, a stomach ulcer, or another bleed continues
- A second deficiency, such as B12, is also present
Those problems are found by history, an examination, and a few directed tests. Adding intravenous iron before that search can hide the diagnosis and, if the person is not iron deficient, add iron they do not need.
Which reports suggest this is not iron deficiency?
A normal ferritin, a blood film that mentions abnormal cells, a very small red-cell size that fits thalassemia trait, low white cells or platelets beside the anemia, or hemoglobin that falls despite treatment. Thalassemia trait is common and does not improve with iron unless true iron deficiency is also present. Aplastic anaemia and other marrow disorders need a different pathway entirely.
What should the hematology visit achieve?
You should leave knowing whether iron is actually low, whether a bleed needs another specialist, and whether the film needs a marrow test. Bring the original counts, the iron studies, and a note of how many tablets were really taken. Dr. G. Stitha Pragna does this review at AOI Hospital, Nallagandla, and S&S Clinic, Manikonda.
Questions patients ask
How long should iron be tried before calling it a failure?
If the diagnosis is iron deficiency and the tablets are taken, hemoglobin often starts to rise within a few weeks. No rise after about four weeks, or falling hemoglobin, should be reassessed rather than continued blindly for months.
Can the problem be absorption or ongoing bleeding?
Yes. Missed doses, tablets taken with tea, stomach or bowel disease, heavy periods, and hidden bleeding all keep the count down. Those causes need treatment. A bigger iron dose does not fix them.
Which anemias are not iron deficiency?
Thalassemia trait, B12 or folate deficiency, anemia of chronic disease, kidney disease, hemolysis, and bone marrow disorders can all look like “low hemoglobin.” The indices and the film usually separate them.
