Aplastic Anaemia: When Tiredness, Infections, and Bleeding Need Urgent Review in Hyderabad
Why aplastic anaemia is different from ordinary anemia, and when low counts cannot wait.
Quick answer. Aplastic anaemia is bone marrow failure, not a simple lack of iron. The marrow stops making enough red cells, white cells, and platelets. Severe tiredness, fever, and bleeding can appear together. It needs prompt specialist care. Treatment may include immune therapy or a stem cell transplant for suitable patients.
People say “anemia” for any low hemoglobin. Aplastic anaemia is a different problem. The bone marrow factory itself has slowed or stopped, so red cells, white cells, and platelets all fall. Iron, vitamin B12, and a single transfusion do not rebuild that factory.
Dr. G. Stitha Pragna sees this when a person has been given repeated iron courses while infections and nosebleeds were treated as separate illnesses.
Which three problems appearing together should not be split up?
- Tiredness and breathlessness from a falling hemoglobin
- Fever or repeated infections from low white cells
- Nosebleeds, gum bleeding, or skin spots from low platelets
Any one of these can have a simpler cause. All three, or two of them on one blood count, need a hematologist rather than another week of iron.
How is the diagnosis made?
The blood count is the clue. The bone marrow test is the confirmation. It also checks that the empty marrow is not leukemia, which can look similar in the blood. Until that result is back, fever and bleeding are managed as urgent, because waiting for a label does not make a low neutrophil count safe.
What does treatment depend on?
Age, how low the counts are, and whether a donor is available decide between immunosuppressive treatment and a stem cell transplant. Transfusions support the gap. They are not the cure. Details of that choice are on the aplastic anaemia page. A fever or active bleeding before the appointment is a reason to go to hospital the same day, then continue the plan at AOI Hospital, Nallagandla, or S&S Clinic, Manikonda.
Questions patients ask
Is aplastic anaemia the same as iron deficiency?
No. Iron deficiency lowers hemoglobin and is often treated with iron. Aplastic anaemia lowers red cells, white cells, and platelets because the marrow is empty. Iron tablets do not fix it.
How is it diagnosed?
Blood counts show low cells of more than one type. A bone marrow examination shows an empty marrow and excludes leukemia. The diagnosis is not made from tiredness alone.
What treatments are used?
Depending on age, severity, and donor options, treatment may be immunosuppressive medicine or a stem cell transplant. Supportive transfusions and infection care happen alongside that choice. The plan is individual.
When is it an emergency in Hyderabad?
Fever with very low white cells, or bleeding with very low platelets, needs same-day hospital care. Dr. G. Stitha Pragna then follows eligible patients at AOI Hospital, Nallagandla, and S&S Clinic, Manikonda.
